Summer School Waiver and Release of Liability

Waiver and Release
I, the undersigned parent/guardian, hereby grant permission for my child to participate in the Summer School Program
organized from July 6, 2026, to July 24, 2026, held on weekdays from 9:30 AM to 3:00 PM. I acknowledge that this
program includes a variety of indoor and outdoor activities, including but not limited to sports, games, trampoline parks,
picnics, barbecues, swimming, and group outings, all of which involve inherent risks of injury.
By signing below, I, the undersigned parent/guardian, agree to the following:

1. Assumption of Risk: I understand and voluntarily accept all risks associated with participation in the
   program’s physical, recreational, and off-site activities.
2. Release of Liability: I, on behalf of myself and my child, release and discharge the program organizers,
   coordinators, mentors, volunteers, and all affiliated personnel from any claims or liabilities for any injury,
   damage, or loss that may occur during the program, except in cases of gross negligence or misconduct.
3. Medical Treatment Authorization: I authorize program staff to seek emergency medical treatment for my
   child if necessary and accept full responsibility for any resulting medical expenses.
4. Off-Site Activity Permission: I give permission for my child to participate in supervised off-site trips and
   activities (e.g., trampoline parks, swimming, picnics, and similar outings).
5. Photo/Video Release: I allow my child’s photo or video to be used in program-related promotional,
   educational, or documentation materials without compensation.
6. Compliance and Conduct: I understand that my child is expected to follow all instructions given by mentors
   and staff and comply with all program rules. I acknowledge that failure to do so may result in disciplinary action,
   including dismissal from the program.
   Program Rules
   ● Cell phones will be collected and securely stored by mentors and returned during designated times only. Other
   electronic devices, if brought, may be held for the duration of the day.
   ● Any damage to facilities, equipment, or property must be compensated by the individual(s) responsible.
   ● Medication sharing is strictly prohibited. All medications must be reported to a mentor upon arrival.
   ● Personal belongings should not be left unattended in common areas. Staff are not responsible for lost or
   misplaced items.
   ● Respectful language and behavior are required at all times. Profanity or inappropriate conduct will not be
   tolerated.
   ● Leaving the program premises or activity area without mentor supervision and permission is strictly prohibited.
   Acknowledgment: I acknowledge that I have read and understood this waiver and consent form in its entirety and
   agree to its terms voluntarily.
